Apply online through the Schools of Public Health Application Service (SOPHAS) online application form.

Deadline

The priority deadline for the receipt of completed MS program applications is May 1.

Priority deadline for diversity funding is January 15.   

Please take into account that it takes approximately four weeks from the date that a SOPHAS application is completed for the Department of Community Health and Health Behavior to receive the application.

Applications are considered for fall semester admission only; there are no spring semester admissions.

Three-step Application Process

  1. First, complete the SOPHAS online application form.
  2. When completing the SOPHAS application, please be sure to address the following in your personal statement: 
  3. Finally, pay the University at Buffalo application fee. The fee is $50 and should be paid through the UB ePay website.

Please note that the $50 UB application fee is different from the application fee paid to SOPHAS.
