Rehabilitation of patients with osteoarthritis (OA) is typically
based on home exercise programs. These programs are believed to
benefit patients and have been shown to qualitatively improve strength.
The purpose of the present study was to quantitate the effects of
a three-month home exercise program on muscle function and functional
capacity. Nineteen patients with OA of the knee began the program,
but only nine completed it. The subjects were aged 65 to 75 years
and weighed 86.16 +/- 12.93kg. The subjects initially had significantly
reduced muscle and functional capacity. Maximal isometric strength
of knee extension increased significantly at a knee angle of 43
degrees for hip angles of 120 degrees, 90 degrees, 60 degrees, and
150 degrees (35%); however, it failed to increase at longer muscle
lengths. There were no significant improvements in hamstring strength.
Maximal speed of contraction against all resistances improved significantly
after three months of exercise (40%). Muscle endurance of the quadriceps
did not improve significantly. Functional capacity did not improve
significantly after the program. These data fail to demonstrate
improved function in OA patients after home exercise, in spite of
improved quadriceps strength and speed. The absence of improvement
in function may reflect the failures of strength, at the longer
muscle lengths, and endurance to improve.
