Osteoarthritis (OA) of the knee is a major health problem leading
to loss of function and disability. This study was carried out to
evaluate the effectiveness of a new muscle rehabilitation program,
which was progressive and quantitative in nature, on muscle and
functional performance in patients with OA (n = 30, age = 62.8 +/-
10 years). The subjects exercised three times per week for three
months on specially designed equipment. Maximal isometric strength
of leg extension increased from 13 +/- 1 to 15 +/- 1kg. Maximal
isometric knee flexion strength increased from 5 +/- 1 to 7 +/-
1kg. Quadriceps and hamstring fatigue (force-time integral) increased
from 633 +/- 120 to 971 +/- 131 and 190 +/- 28 to 299 +/- 13 kg/sec,
respectively. The increases in muscle function were associated with
increased muscle length, but not with muscle volume. Walking time
decreased from 11.2 +/- 3.1 to 8.7 +/- 1sec. Dependency difficulty
and pain improved from 1.2 +/- 0.2 to 1.19 +/- 0.1, 2.2 +/- 0.6
to 1.7 +/- 0.5 and 1.7 +/- 0.5 to 1.4 +/- 0.3, respectively, as
determined by the Jette Functional Status Index. Progressive quantitative
muscle rehabilitation significantly increases both muscle function
and functional performance. This type of program would appear useful
for nonsurgical treatment in patients with OA.
