Changing Concentrations
An MPH student who wishes to change concentrations must first apply to and be accepted by the prospective department. The student should request to have their original application file forwarded from their current department to the department to which they are applying. There is also a form that needs to be completed, the Request to Change MPH Concentration.

Graduation Deadlines
There are department deadlines and graduate school deadlines. Department deadlines precede graduate school deadlines. Check with your department!

ATC - Application to Candidacy and Abstract of Proposed Integrative Project
The Graduate School web site provides the ATC form (word doc).
The "committee" - with the advice of
their advisor the student selects a committee which consists of
a major professor who is a member of the Graduate Faculty (a list
of eligible Graduate Faculty is available at the Graduate School
website at
http://www.grad.buffalo.edu/academics/facultyroster/roster.cgi)
and at least one other faculty member who holds the rank of assistant,
associate or professor in the University but need not be a member
of the Graduate Faculty. Often, the committee consists of the student's
faculty advisor for their integrative project as the major professor
and another faculty member (often the Director of Graduate Studies).
MPH students may be required by their department to submit an abstract of their proposed MPH integrative project with the ATC form or shortly after the start of the semester during which the student is registered for the integrative project (SPM 630 or STA 630).

M-Form
The M-form is submitted to the Graduate School to certify that the integrative project was satisfactorily completed and that ALL academic requirements for the MPH degree have been satisfied. It is available from your Department at or about the time you are completing your integrative project. MPH students will need to submit along with the M-form a 150 words or less summary (abstract) of what they did for their integrative project. Student name and project title should be at the top of the summary (abstract).

Health - click on “student" link. Typically, the
summer program officially begins May 1 and ends September 30th.
Students should plan to spend a minimum of 8 weeks during this
interval. Start and ending dates are flexible and unpaid vacations
can be arranged. Online applications are due by March 1st, but
it is always good to complete applications sooner rather than
later.
- Students with an expressed interest in reproductive, perinatal or pediatric endpoints or methodologies pertinent to etiologic or clinical prediction research in this area are most welcome to apply to National Institute of Child Health & Human Development. Information about the types of research conducted by the Division of Epidemiology, Statistics & Prevention Research should be available at the NICHD’s website. Given the number of applications that NIH receives, students should email Dr. Germaine M. Buck Louis (Senior Investigator & Chief, Epidemiology Branch; Division of Epidemiology, Statistics & Prevention Research - louisg@mail.nih.gov) or other investigators in the Division upon submission of an application.

- Pfizer: Briefly, this program is
to develop good public relations for Pfizer, introduce students
to the drug research and development, create relationships with
universities and create a long-term talent pipeline for Pfizer.
Graduate students will be assigned specific project with associated
staff supervisors and mentors and given a weekly stipend of about
$700 for 10-12 weeks. There are over 80 positions available within
the Groton/New London, CT campus and three specifically opened
in the Dept. of Outcomes Research (epidemiology, health policy,
