Exercise and Nutrition BS/MS
Application Information
(to be completed once you are accepted into the upper division)
- APPLICATION FORMS must be filled out on-line for students accepted into the upper division. Students should apply to the BS/MS Program by using the Graduate Application site . The on-line application can be filled
out in stages (you can use cut and paste to fill out the statements).
Online applicants must obtain recommendation forms, which can
be downloaded below.
Foreign students must submit a copy or fax (716-829-2428)
of their biographical page (with picture) from their passport.
- STATEMENT OF CAREER OBJECTIVES AND PLANS (included
in online application) should include your area of concentration
and research interests. This section is very important in the
application process and a lack of detail may cause a delay in
processing your application materials.
- CONTACT A FACULTY MEMBER in your area of interest
prior to submitting your application materials, if you are applying
for the thesis option. An interview (in-person or by phone) should
be arranged with a faculty member from the area of concentration
you have chosen. For a listing of the

- APPLICATION FEE The State has approved new
application fees for selected programs. Fees for programs in SPHHP
are now $50 for electronic filing. Please make check payable to:
The State University of New York at Buffalo. This needs to be
sent to the Graduate Secretary (see below). You may also pay using
a credit card.
- TWO RECOMMENDATION FORMS (Word) must
be sent to the Graduate Secretary. These forms should be supplied
by individuals who have worked closely with you and have more
than casual knowledge of your academic performance. Please use the admissions form (2 each) if only applying to the program and the Graduate Assistantship form (2 each) if interested in assistantships.
- OFFICIAL TRANSCRIPTS must be requested from every institution you have attended (including the University at Buffalo) and sent to the Graduate Secretary.
- TOEFL scores of greater than 549 on the written
test or 212 on the computer-based test are required for students
who are not native speakers of English, including those who are
transferring from or who have completed degree programs at other
US colleges or universities. The ETS code for UB is 2925,
for Exercise Science is 0619 and for Nutrition is 0214.
