Exercise and Nutrition BS/MS

Application Information

(to be completed once you are accepted into the upper division)

 

  • APPLICATION FORMS must be filled out on-line for students accepted into the upper division. Students should apply to the BS/MS Program by using the Graduate Application site . The on-line application can be filled out in stages (you can use cut and paste to fill out the statements). Online applicants must obtain recommendation forms, which can be downloaded below. Foreign students must submit a copy or fax (716-829-2428) of their biographical page (with picture) from their passport.
  • STATEMENT OF CAREER OBJECTIVES AND PLANS (included in online application) should include your area of concentration and research interests. This section is very important in the application process and a lack of detail may cause a delay in processing your application materials.
  • CONTACT A FACULTY MEMBER in your area of interest prior to submitting your application materials, if you are applying for the thesis option. An interview (in-person or by phone) should be arranged with a faculty member from the area of concentration you have chosen. For a listing of the graduate faculty and their interests click here.
  • APPLICATION FEE The State has approved new application fees for selected programs. Fees for programs in SPHHP are now $50 for electronic filing. Please make check payable to: The State University of New York at Buffalo. This needs to be sent to the Graduate Secretary (see below). You may also pay using a credit card.
  • TWO RECOMMENDATION FORMS (Word) must be sent to the Graduate Secretary. These forms should be supplied by individuals who have worked closely with you and have more than casual knowledge of your academic performance. Please use the admissions form (2 each) if only applying to the program and the Graduate Assistantship form (2 each) if interested in assistantships.
  • OFFICIAL TRANSCRIPTS must be requested from every institution you have attended (including the University at Buffalo) and sent to the Graduate Secretary.
  • TOEFL scores of greater than 549 on the written test or 212 on the computer-based test are required for students who are not native speakers of English, including those who are transferring from or who have completed degree programs at other US colleges or universities. The ETS code for UB is 2925, for Exercise Science is 0619 and for Nutrition is 0214.
