Please allow sufficient turn around time for application deadline. - DIETETIC INTERNSHIP CENTRALIZED APPLICATION SERVICES (DICAS) participation is required.
This service will provide your application to the SUNY at Buffalo Dietetic Internship, along with three recommendation forms, unofficial transcripts, verification statements or intent to complete degree and personal statement of career objectives and plans. Application materials may be obtained through any ADA approved Didactic Program in Dietetics or from DICAS at http://portal.dicas.org (customer support number 617-612-2855). Please allow sufficient turnaround time for application deadline. - APPLICATION TO NUTRITION ADVANCED CERTIFICATE (NAC) - Select Nutrition Advanced Certificate from the drop down list. Make sure to include your electronic signature! (Otherwise your application will be listed as incomplete!)
- TWO application fees - The fee for the internship application is $50.00 in US funds, made payable to: The UB Foundation. It should be sent directly to the Program Director. Please pay the Nutrition Advanced Certificate $50 application fee using a credit card with the online application process.
- VERIFICATION STATEMENT with an original signature of your Didactic Program Director. IF you have not completed all course work, then a Declaration of Intent to Complete the Degree and /or ADA Approval of Minimum Academic Requirements signed by your Didactic Program Director (DPD). Final original documents can be sent to the Program Director after acceptance. (Unoffical documents will be sent through DICAS.)
- OFFICIAL TRANSCRIPTS must be requested from every institution (including University at Buffalo) you have attended and sent to the Program Director, to show a minimum overall GPA of 3.0 for undergraduate work. Official transcripts can be sent to the Program Director after acceptance. Official transcripts must also be sent with the Nutrition Advanced Certificate application after acceptance. Final transcripts must reflect BS degree conferral, sent to both the Dietetic Internship and the Graduate program. (Unoffical documents will be sent through DICAS.) - GRE scores are required for acceptance into the Nutrition Advanced Certificate.
GRE's do not have to be included in the application packet but must be completed prior to the start of the internship (August 1st). The ETS code for UB is 2925, Exercise Science is 0619 and Nutrition is 0214. The GRE web site has additional information. - Foreign Students must request additional forms.
Foreign students from non-English speaking countries must also take the TOEFL exam. The TOEFL score of greater than 549 on the written test or 212 on the computer-based test for students who are not native speakers of English, including those who are transferring from or who have completed degree programs at other US colleges or universities, is required.
