be a dietitian, why you want to come to this program, and what
strengths you would bring to this program. This is important in
the application process and a lack of detail may cause a delay
in processing your application materials. You do not need to fill
this section out on the graduate application, but do need to submit
this document with your internship application.

course work, then a Declaration of Intent to Complete the Degree
and /or ADA Approval of Minimum Academic Requirements signed by
your Didactic Program Director.
- THREE RECOMMENDATION FORMS (PDF or Word) be
sent to the Internship Director. Both the internship and graduate
programs will share recommendations. You only need to send three.
These can be the same as those used for the MS application. These
forms should be supplied by individuals who have worked closely
with you and have more than casual knowledge of your academic
performance.
- OFFICIAL TRANSCRIPTS must be requested from
every institution you have attended and sent to the Internship
Director, including the University at Buffalo, to show a minimum
overall GPA of 3.0 undergraduate and/or graduate
course work. Both programs will share one set of transcripts.

the MS program. GRE’s do not have to be included
in the application packet but must be completed prior to the start
of the internship. The ETS code for UB is 2925, Exercise
Science is 0619 and Nutrition is 0214. The GRE web site has additional information.
- Foreign students
must request additional forms. Foreign students
from non-English speaking countries must also take the TOEFL
exam. The TOEFL score of greater than 549 on the written test
or 212 on the computer-based test for students who are not native
speakers of English, including those who are transferring from
